In consideration of Anderson’s Outdoor Adventures LLC rental services, The Anderson’s Outdoor Adventures LLC (hereafter referred to as AOA) premises and/or equipment to enable me to participate in kayaking, canoeing, paddle boarding, river tubing, pontoon boating, RV usage, golf cart usage, and shuttle services I agree as follows:

Golf Cart Liability Waiver and Rental Agreement

By Signing Below I acknowledge the following statements

• I acknowledge I am at least 21 years of age and possess a valid Driver’s License in 1 of 50 states.

• I acknowledge no one under the age of 16 is authorized to operate the golf cart at any time and that those who are 16 must possess a valid driver’s license in 1 of the 50 states.

• I acknowledge that I am bound by the rules and regulations of this agreement, Anderson’s Outdoor Adventures, LLC (AOA) , and the establishment/park I am operating the Golf Cart in or on. 

• I acknowledge that the max weight capacity of all passengers and  cargo can not exceed 750 lbs per cart and that I am responsible for any and all damages that may occur from not complying with weight limits.

• I acknowledge that run times will vary based on terrain, weight of passengers and cargo and  Golf Cart lights at night. 

• I acknowledge that any modifications to the Golf Cart are prohibited. 

• I agree to contact AOA at (352) 507-0059 with any questions about operation or charging of the Golf Cart. 

• I acknowledge I have been instructed on how to and when to charge the Golf Cart. 

• I agree to inspect the safety and mechanical condition of the Golf Cart prior to each use and to report any issues to AOA immediately at (352) 507-0059

• I agree to return the cart in the same condition as received minus normal wear due to authorized use of the Golf Cart and that the golf cart is not designed for rough terrain or off road use. 

• I acknowledge and agree that the privilege of operating the Golf Cart can be revoked and rental agreement cancelled without refund with any violation of the rental agreement or in violation of the park/venue rules.

Each person in a group must submit one of these forms. If the person is under the age of 18 then a parent or guardian must sign their form and assume responsibility for the minor. 

In consideration of Anderson’s Outdoor Adventures LLC rental services, The Anderson’s Outdoor Adventures LLC (hereafter referred to as AOA) premises and/or equipment to enable me to participate in kayaking, canoeing, paddle boarding, and river tubing I agree as follows: 

I fully understand and acknowledge that: 

(a) outdoor recreational activities have inherent risks, dangers, and hazards and such exist in my use of paddle sport equipment and my participation in paddle sport activities. (B) my participation in such activities and/or use of such equipment may result in injury or illness including, but not limited to bodily injury, disease, strains, fractures, partial and/ or total paralysis, death or other ailments that could cause serious disability; (c) these risks and dangers may be caused by the negligence of the owners, employees, officers or agents of AOA; the negligence of participants, the negligence of theirs, accidents, beaches or contract, the forces of nature or other causes. Risks and dangers may arise from foreseeable or unforeseeable causes including, but not limited to, guide decision making, including that a guide may misjudge terrain, weather, trail or river conditions, and water level, risk of falling out of or drowning while paddle sporting and such other risks, hazards and dangers that are integral to recreational activities that take place in a wilderness, river, outdoor, or recreational environment. 

By my participation in these activities and/ or use of equipment, I hereby assume all risks and dangers and all responsibility for any loses and/ or damages, whether caused in whole or in part by the negligence or other conduct of the owners, agents, officers, or employees of AOA, or by any other person including myself. I also verify that I am in good health, Not under the influence of any drugs or alcohol of any kind, nor do I have them in my possession , and am capable of pursuing paddle sports, and that my participation in this activity is voluntary. I/we also agree to a properly stowed or worn USCG approved personal floatation device, at all times. 

I, on behalf of myself, my personal representatives and my heirs, hereby voluntarily agree to release, discharge, hold harmless, defend and indemnify AOA and its owners, officers, employees, lessors, contract mangers, Department of Environmental Protection, and the Board of Trustees, from any and all claims, actions or losses for bodily injury, property damage, wrongful death, loss of services or otherwise which may arise out of my use of paddle sport equipment or premises or my participation in paddle sport activities. I specifically understand that I am releasing, discharging, and waiving any claims or actions that I may have at present time or in the future for the negligent acts or other conduct by the owners, agents, officers or employees of AOA.
